Delivery Challan without pricing format

Sometimes i will ask my supplier to deliver the goods directly to my customer with my delivery challan, i dont want my supplier to know my pricing which i have given to my customer. The format required would be ship to, bill to and item name, item description and quantity, bank details are not required .
